On Sunday, about 4 a.m., an Audi convertible collided on the A460 Cannock Road near the intersection of Wood Hayes Road in Westcroft, Wolverhampton.
The Independent Office for Police Conduct (IOPC) has begun an investigation, stating that the collision involved one automobile and occurred following a “police pursuit”.
Wood Hayes Road and the A460 Cannock Road were closed on Sunday morning as an inquiry into the event was launched.
The West Midlands Ambulance Service reported on Monday that two individuals died in the incident and a third was critically injured.
The grey Audi left the road and became entangled in a hedge along the A460, the main road connecting Wolverhampton and Essington.
It sustained significant front-end damage, including a cracked window, and was recovered on Sunday lunchtime, with leaves and branches still stuck in the vehicle.
An unmarked Audi police car was parked nearby behind the cordon, where a blue screen was set up while collision investigators assessed the scene.
Emergency services arrived early in the morning, waking up some homes on Cannock Road near the Staffordshire border.
There are no recommendations at the moment.
Neighbours expressed their shock and sadness upon learning that two persons had perished.
David Ray, 73, stated: “The flashing lights startled me up. I did not hear a bang. There were numerous police officers present, as well as an ambulance.
“It’s a shame; it’s bad. We do speed around here, but we haven’t had an accident in four or five years.”

Dave and Karen Foster reside near the police barrier that remained in place on Sunday afternoon.
Mr Foster stated, “It’s dreadful. You do not expect it. We do get speedy around here.”
Wife Karen stated, “It’s upsetting; you don’t want to hear it.” It’s very disturbing.
Another resident, who did not want to be identified, stated, “I slept through it.” I got up this morning and saw the police cruiser, and I knew I wouldn’t be leaving for a while.”
An IOPC spokesman said: “We have launched an independent investigation into the circumstances surrounding a road traffic incident on Wood Hayes Road, Essington, Wolverhampton, at 4 a.m. this morning, which followed a police pursuit.”
“The unfortunate collision involved one vehicle, and two people were tragically killed at the spot.
“Our investigators have been assigned to the post-incident procedures and are conducting an examination of the facts and available information.
“Our investigation follows a mandatory referral from Staffordshire Police and is in its early stages.”
The A460 was closed in both directions from Old Hampton Lane to Wood Hayes Road on Sunday morning.
A Staffordshire Police spokeswoman said: “Staffordshire Police attended to a major collision on Wood Hayes Road, Essington, at around 3.50 a.m. today.
